-I 13 -g 2 -w 256 --thread-concurrency 8192

Try with that + ip , port , username , password .

When i put my Thread concurrency to that, cgminer crashes..

Then use another thread concurrency , i was sitting 2 days on that to find the best settings for my card.

Trial and error !!!
